End of Year Program Evaluation

Apprenticeship

Clarity of Calling

One of the primary goals of the Planted Apprenticeship is to test, confirm, or discover your ministry calling. With that in mind, please answer the next few questions.

Head, Heart, and Hands

Planted is based around the three-part discipleship model of head, heart, and hands. Please reflect on how the past year of apprenticeship has helped you develop in these three areas.

Ongoing Education

Planted utilized reading and round table discussions to engage in ongoing education in topics relevant to ministry. Think about the books, topics and discussions as you answer the questions in this section.

Peer-to-peer Discipleship

Planted utilized peer discipleship to foster a focus on prayer, confession of sin, accountability, and the spiritual discipline of Scripture reading. Consider this element of the program as you answer the questions in this section.

Mentored Practice

Planted used the Mentored Practice model to transition you from observation to independent ministry. Please reflect on your experience with this model in the apprenticeship program.
